Twenty one chronic heart failure patients will be recruited from the Heart Failure clinic as Eastbourne District General Hospital according to the inclusion and exclusion criteria and procedures outline below:-
Participant Inclusion Criteria
• Systolic heart failure with resting left ventricular ejection fraction (LVEF) < 40%
• New York Heart Association (NYHA) class I–III
• clinically stable for at least 4 weeks on optimised medication dosage according to current guidelines
Participant Exclusion Criteria
Summary of contraindications to exercise testing and training, and increased risk for exercise training, European Society of Cardiology (Piepoli et al., 2011).
• Acute coronary syndrome within past 6 months
• Untreated life-threatening cardiac arrhythmias
• Acute heart failure (during initial period of haemodynamic instability)
• Uncontrolled hypertension
• Advanced atrioventricular block
• Acute myocarditis or periocarditis
• Symptomatic aortic stenosis
• Sever hypetrophic obstructive cardiomyopathy
• Acute systemic illness
• Intracardiac thrombus
• Progressive worsening of exercise tolerance of dyspnoea at rest over previous 3-5 days
• Significant ischaemia during low intensity exercise (< 2 METS, < 50W)
• Uncontrolled diabetes
• Recent embolism
• Thrombophlebitis
• New onset atrial fibrillation/flutter
• > 1.8 kg increase in body mass over previous 1-3 days
• Concurrent, continuous or intermittent dobutamine therapy
• Decrease in systolic blood pressure with exercise
• NYHA Functional Class IV
• Complex ventricular arrhythmia at rest or appearing with exertion
• Supine resting heart rate > 100 beats.min-1
Patients meeting the inclusion and exclusion criteria will be identified by a review of medical records by the direct healthcare care team onlyThis is a single centre, prospective, observational study. Following recruitment, study participants will be requested to perform an exercise test to define individual exercise intensity thresholds. Participants will then join the existing Phase III hospital-based cardiac rehabilitation programme. On completion of Phase III cardiac rehabilitation, participants will repeat the exercise testing session to redefine individual exercise intensity thresholds before progressing to the Phase IV programme at the University of Brighton Sports Centre, Eastbourne. Expired gas analysis and affective responses will be measured during one exercise session from each programme. Weekly physical activity will be measured via accelerometry for a one week period during each programme |
